2nd Semester 2009-2010:
November 9 – 10, 2009

Enrollment Procedure
STEP 1
Go to Table 1 and get Registration Form.
For Graduating Students: get Application for Graduation.
For Old Students: Ask person in charge if you have unsettled accounts.
STEP 2
Meet with advisor/Program Director. Fill-out Registration Forms.
Secure advisor’s signature on your Registration form.
STEP 3
Go to Table 2 and secure Registrar’s Signature on your Registration Form.
STEP 4
Go to Table 3 for Financial Assessment.
For New Students: Please inform the Officer in Charge regarding your application fee.
Show receipt of Payment if settled already.
STEP 5
Go to Business Office for payment. Make sure your Registration Form is stamped PAID.
STEP 6
Go back to Table 1. Submit White and Green copy of your Registration Form. Keep the
Blue copy.
STEP 7
Go to Table 4 and fill out Application Form for Computer Usage.
New Students: Apply for ID.
Old Students: Proceed to the Academic Dean for ID validation.
STEP 8
Sign-up for Growth Group and for your Book Orders. Proceed to the library and
present your Blue copy for validation of your Library status.
